• The Family Supports home- and community-based services waiver (FSW) provides limited, non-residential supports to individuals with developmental disabilities who live with their families or in other settings with informal supports.

    Individuals must meet HCBS waiver eligibility and Medicaid eligibility guidelines in order to be eligible for a Medicaid HCBS waiver. To be eligible individuals must:

    • Be diagnosed as having an intellectual disability prior to the age of 22

    • Reside in or be transitioning into an HCBS-compliant setting (non-institutionalized)

    • Have income no greater than 300% of the maximum Supplemental Security Income amount (parental income for children under 18 years of age is disregarded)

    • Meet Intermediate Care Facility for Individuals with Intellectual Disabilities/Development Disabilities (ICF/IID) level of care

  • The Community Integration and Habilitation (CIH) waiver provides services that enable individuals to remain in their homes or community-based settings. The CIH waiver also assists individuals who are transitioning from state-operated facilities or other institutions into community settings. Home and community-based services (HCBS) through the CIH waiver allow flexibility in providing the supports necessary to help individuals gain and maintain optimum levels of self-determination and community integration.

Meet the Team

Nicole Gilmore, MA & RBC - Title: Director of Waiver Services

Nicole began her career in 1998 at Hillcroft as individuals were being deinstitutionalized and transitioned to the community. She met Dr. CJ Gallihugh at Hillcroft and later followed her when she started Behavioral Interventions, Inc. in 1999. Nicole received her master's degree from Ball State in 2006 in Special Education. Behavioral Interventions dissolved in 2021 into Insights/Via-quest. In September of 2023, she went to the Youth Opportunity Center to assist in a new Therapeutic Day School in partnership with the Muncie Community Schools. She learned a good deal, but realized her heart was with the Waiver. Nicole joined the HBS team in January, heading up our behaviorists and carrying a small caseload.

Beverly Forrey, MA - Title: Behaviorist

Beverly has over 19 years of experience working with individuals on the Autism Spectrum and related disabilities. She began her career in Applied Behavior Analysis (ABA) in 2005 as a Behavior Technician and earned her Master of Arts in Special Education from Ball State University in 2010, with certificates in Autism and ABA. That same year, she founded Hope Behavioral Services, LLC, which has grown while maintaining her commitment to clients and families. In 2012, Beverly also began serving as a Behavior Consultant with the Indiana Medicaid Waiver Program. She has worked with children and adults across clinic, home, and community settings, and continues to balance clinic operations with direct consulting. Her dedication to supporting individuals and families is central to her work.

Outside of her profession, Beverly enjoys time with her husband Greg, their two sons Shawn and Hunter, as well as traveling and being with friends.

Delaney Smith, MA - Title: Behaviorist

Bio- Delaney grew up with 5 younger adopted siblings all of whom had special needs. She volunteered in the life-skills classroom throughout school. Delaney received her bachelor's degree from IUK in Biological Science Psychology with a focus in General Sciences in 2021, then received her master's degree from Ball State in Applied Behavioral Analysis with a certification in Autism Spectrum Disorder in 2023. She has been working in ABA for the past 5.6 years and began working as a behaviorist on the waiver in 2024. Delaney’s passion is helping her clients access fulfillment of their needs, advocation for those who cannot advocate for themselves, and assisting her clients to develop independence and autonomy. All clients deserve to love the life they live, and so do their families

Caitlin Smith, MA - Title: Behaviorist 

Bio-Caitlyn started working with individuals with autism in 2020. Working with her client and seeing them succeed gave her the push she needed to finish her degree at IUK with a Bachelors in Science of Psychology, then go on to Ball State to get her Masters in Applied Behavioral Analysis. She loves to see the progress that her clients make and help families to have a voice. Caitlyn began working on the waiver in 2024.

Staci Duncan, MS - Title: Behaviorist 

Bio-Staci has been working with individuals with special needs for 25 years. She has been a special education teacher, first step provider, special eduction director, and is currently a middle school assistant principal. Special education has been her primary focus. She received her undergrad at IU in Elementary Education & Master’s degree is in Special Education. She enjoys working with all ages of clients and feels parent training is a very important part of her job. She has been working with individuals on the waiver since 2023.

Jessey Holcomb, LSW - Title: Behavioralist, LSW

Bio- Jessey has been working with youth and adolescence with various disabilities for over 16 years . He received his Bachelor’s degree from Liberty University in Elementary Education, then Master’s degree from Indiana Wesleyan University in Social Work. Jessey has experience working with clients on anger, impulse control, elopement and task refusal. For Jessey, the best part of working with his clients is getting to be hands on and helping them to achieve a solution. He has been working with the waiver since 2023

Kim Helton, MSW, BSW, MA - Title: Behaviorist

Kim began her social work career in 1995 as a Youth Care Specialist with the Children’s Bureau of Indianapolis after earning her bachelor’s in social work from Slippery Rock University. She later provided home-based counseling to children and families before relocating to Kokomo, where she spent 25 years as a social services director in long-term care. In 2009, Kim earned a Master of Arts in Gerontology from Ball State University and, in 2022, completed her master’s in social work at Indiana Wesleyan University. Her recent experience includes mental health, special needs, and healthcare consulting. As a parent to a child on the autism spectrum, Kim deeply values advocacy, emotional regulation, and treating individuals with compassion and respect. She is passionate about helping people of all ages grow and thrive. Kim joined the HBS team in July 2025.

Alicia Sandoval, MA - Title: Behaviorist

Alicia joined Hope Behavioral Services in 2024. She holds a bachelor’s degree in Psychology and a bachelor’s in Sociology from IUPUI, and earned her master’s degree in Applied Behavior Analysis from Ball State University in 2025. She currently works as an RBT and BCBA intern, and recently began providing services under the Indiana Medicaid Waiver. She believes that the best part of the job is seeing progress over time and being part of the small changes that can make a difference. Outside of work, she enjoys playing tennis and engaging in artistic activities like painting, scrapbooking, and photography.
